Clinic and Financial Policies

At Holland Biomedical Clinic, we are passionate about what we do, and we feel that we have a calling to provide as many people as possible with the highest quality natural health care possible. Just as our services are unique to this region, our financial policies set us apart from mainstream medicine.  

Patients and/or their parents/legal guardians should read each policy section listed below. If you have any questions, please contact the clinic at 952-215-3767 or by email at patientsupport@hollandcenter.com. When you make your first appointment and annually thereafter, we will send an electronic copy of the policy to review and accept. Until the electronic copy has been reviewed and accepted, your appointment will be tentative and may be cancelled at the clinic’s discretion.

Consultations

The length of consultations vary based the type of consultation and the patient’s specific situation. Below is the amount of time we block for each type of consultation. To get the most out of your appointment, please complete all questionnaires sent you you via the Patient Portal at least 48 hours in advance of your appointment.

  • Initial Functional Medicine Consultation – 1-1.5 hrs

  • Follow-up Functional Medicine Consultation – 0.5-1.0 hrs

  • Hyperbaric Oxygen Therapy Consultation – 0.5 hrs

  • Initial Nutrition Consultation - 1 - 1.5 hrs

  • Follow-up Nutrition Consultation - 0.5 - 1.0 hrs

  • Health Coaching - 0.5 - 1.0 hrs

Consultation/Coaching Rates by Practitioner

  • Anne Kelly, M.D.: $400/hour

  • Catherine Morud, RN CNP: $280/hour

  • Ashley Oswald, RDN, IFNCP, CNSC, CLT, LD: $150/hour

  • Stephanie Ewals, BS, NTP: $100/hour

Charges for physician billed consultations are billed per hour (in 15 min increments). Charges for non-physician consultations are billed per hour (in 10 min increments). All phone consultations are billed per hour (in 10 minute increments). Time spent in conference calls with other physicians will also be billed at this rate.

Laboratory Testing Costs

Costs for laboratory testing are not included in the provider visit charges. Most laboratory companies we use are set up for you to provide them with payment. However, a few companies require the clinic to pay them directly, in those situations we will bill your credit card on file and provide an invoice.

Treatment Rates

  • Hyperbaric Treatment: $100 per treatment for 60 minutes at pressure

  • Hyperbaric Oxygen Therapy: $150 per treatment for 90 minutes at pressure

  • Microcurrent Therapy: $75 per 30 minutes treatment

  • Exercise with Oxygen Treatment: $50 per treatment

  • Ionic Footbath: $35 per treatment

  • Microcurrent Facial: $125 per 50 minute

Insurance

The clinic does not accept health insurance. Fees are due at the time of each visit. We accept cash, credit/debit cards and checks. If you would like to submit your own claim to your insurance company for possible reimbursement, please request a superbill be sent to you through our patient portal. There is no guarantee for reimbursement or coverage for any of our services.

Medicare:

The clinic has opted out of Medicare and does not accept Medicare. The patient is responsible for all charges incurred and cannot bill Medicare for the services provided.

Payment:

Payment for all charges is due at the time of your appointment. Check, cash, credit or debit card, or a health savings account card are all acceptable forms of payment.

Regardless of the above payment alternatives, we require every patient to have a valid credit or debit card kept on their account. If payment is not received at the time of your appointment for any reason, the card on file will be charged for products, services rendered or any other clinic fees.

Credit/Debit Card on File:

Appointments for new patients are often booked out weeks in advance due to patient demand to be seen by our clinic staff. If a patient cancels an appointment or fails to show up for a scheduled appointment, we miss an opportunity to have helped another patient.

To minimize these missed opportunities, the clinic requires a valid credit or debit card to be kept on file to schedule a consultation or a treatment. The patient authorizes the clinic to charge the card on file unless another form of payment is provided at the time of service.

If patients prefer to pay with cash or a check, they can do so at the time of service. However, in order to book an appointment, a valid credit or debit card must be on file. There are NO exceptions to this policy.

Cancellation / No Show Fee - Initial & HBOT Consultations

Initial functional medicine and nutrition consultations, as well as Hyperbaric Oxygen Therapy consultations must be cancelled or rescheduled at least 1 week prior to the appointment.

If you do not cancel or reschedule your appointment at least 48 hours prior to your scheduled appointment, the patient authorizes the clinic to charge their card on file a cancellation/no show fee of 50% of the specific provider’s typical appointment fee listed above. This fee will not be refunded, even if you schedule a new appointment unless you cancelled within the timeframe above.

Cancellation / No Show Fee - Treatments & Follow-up Consultations

Existing patients will be charged a $50 fee for all treatments or follow-up consultations which are cancelled with less than 48 hours’ notice. The patient authorizes the clinic to charge their card on file for this cancellation/no show fee. However, the clinic will waive this fee for treatments if they are rescheduled in advance of the original appointment time.

Patient Portal

All patients are required to set up a patient portal account. The clinic uses this portal to communicate with you in a way that ensures the privacy of your health data. This type of communication include items such as: lab orders, lab results, treatment plans, invoices, and messages between the patient and the practice.

Due to HIPAA regulations and to ensure patient confidentiality, individual medical advice will not be given through email. All messages should be sent through the patient portal or by calling the office. We will respond as quickly as possible; however, please understand that because our practitioners’ hours vary, replies may take up to 72 business hours.

Email and/or Text Messages

The clinic sends email and/or text message reminders to patients which do not contain protected health data. Typical messages include appointment reminders, notification of a message in the patient portal, etc. Any email that is sent into the clinic which contains health data will only be responded to through the patient portal.

Lab Kits

Lab kits that require the clinic to pay in advance will be billed to your card on file as soon as the order is placed. Lab kits paid by the patient will require a method of payment provided by you when submitting the sample.

All lab tests require an order from your practitioner. If you feel a specific test might be beneficial, please discuss this with your practitioner during your consultation. If you have had an appointment with your practitioner in the past 12 months, you can also message your practitioner through the portal to see if they will order the test. If needed, the clinic will ship a kit(s) to you. Depending on the lab vendor, there may be a shipping charge. All lab kits will be sent to the patient address on file.

Prescription Refills

All prescriptions should be discussed with your practitioner during your consultation. If you need to have a prescription refilled prior to your next consultation, please allow a minimum of 72 business hours. Prescriptions will not be refilled if you have not seen your practitioner in the past 12 months.

Ordering Supplements

Your practitioner may recommend specific supplements that have been identified due to their purity and efficacy. You will be given options regarding resources of where specific supplements can be ordered. The Clinic carries most supplements we recommend, but you are encouraged to order from the most cost-effective and convenient option for you.

If needed, the clinic is able to have supplements shipped directly to your home. A shipping fee will be required on orders less than $100. In the event we do not carry a recommended supplement, we will provide information on how and where you can order it.

Primary Care Physician

The clinic is a specialty practice concentrating in the areas of Functional Medicine and Nutrition. We are not intended to take the place of your Primary Care Physician/Pediatrician. Your PCP will still be the primary contact for your school physicals, acute illness, emergency care, or hospitalizations. We are willing to work with any PCP/Pediatrician in regards to any of the treatments or testing that are recommended. The practice is not available 24 hours a day, and our practitioners do not have admitting privileges to any hospital.

Emergencies

If you are having a medical emergency, please call your primary physician and/or 911.

Letters

Please allow 10-15 business days for any letters of medical necessity, prior authorization requests or any other associated forms. An associated fee may apply. Please call the office for details.
